It is implied by Izumi that just surviving it is a very serious feat. In Fullmetal Alchemist, performing a forbidden human transmutation will take you to the Gate, where, along with being maimed in some way, you get to look inside and see 'Truth', which, as Izumi stated, looked like hell to her.
